TB Prevention in Tribal Communities: A Model Intervention from Malappuram District
As part of the National Tuberculosis Elimination Program (NTEP), Malappuram district’s achievement in ensuring healthcare services for marginalized communities serves as a model for Kerala’s health sector. Despite significant challenges, a patient-centered approach—ensuring both clinical care and nutritional support for an individual from the tribal community—is gaining widespread appreciation.
Case Background During a TB screening campaign conducted in the Karulai region of Malappuram, a 32-year-old tribal woman named Chathi was identified as CyTB positive. However, she suffered from severe malnutrition, weighing only 27 kg, and her reluctance to visit health centers posed a major challenge to initiating treatment.
The Intervention Under the leadership of District TB Officer Dr. Noona Marja, a specialized multidisciplinary team was formed, including Health Center staff and representatives from the Keystone Foundation and Mahila Samakhya. Recognizing that administering medication during severe malnutrition could be risky, the team prioritized improving her health first by providing nutritional kits and supplements to increase her body weight.
Community Engagement and Follow-up Even when the patient returned to her settlement from the hospital, health workers reached her doorstep to deliver medicines and food. To ensure regular adherence to the medication, a local ‘Tribal Promoter’ was assigned to monitor her progress within the community.
Outcome and Lessons Learned As a result of these efforts, the patient’s weight increased from 27 kg to 33.5 kg, and her TB preventive treatment (TPT) is now progressing successfully. This case proves that TB prevention among tribal populations requires more than just providing medicine; it demands personalized care that understands their unique living conditions.
Ensuring nutritional stability before starting treatment and strengthening community linkages are vital strategies for reducing treatment hesitancy in tribal areas. This initiative stands as a significant milestone in strengthening the bond of trust between the formal healthcare system and the tribal community in Kerala
